How Does Eyelid Surgery Work?

During an upper eyelid surgery, incisions are made along natural folds in the skin in the upper lid. From there, lax skin is tailored out. For the lower eyelid, an external incision is often not necessary. Instead, Dr. Bass makes a transconjunctival incision, which is an incision inside the lower eyelid. The transconjunctival approach allows for the removal of excess fatty tissue which contributes to bags under the eyes. 

Excess skin can be tailored out through small incisions if needed.  Sometimes the fat removed can be reinserted into the tear trough hollows to restore lost volume in this area. Fat grafting can also be used.  If laxity in lower eyelid needs to be corrected, the supporting ligament or musculature of the lower eyelid can be lifted and tightened in processes called canthopexy or canthoplasty.
